Relationship to student (If you are the student, please write self)
Student Name
Student Birth Date
Student ID # (if you do not know the id, please supply homeroom)
Parent/Guardian Full Name
Email Address (email address will be your login to the portal)
Cell Phone Number
Cell phone provider (i.e. Verizon)

If you have more than one child attending Union Public Schools, they can be on the same account. Include the information (from above) for each child in your email.
